Once we have your c.v., we can check your eligibility in the states where you wish to be licensed, advise you about the costs involved, and go over any potential problem issues, such as extra exams certain boards require for certain applicants. We will then return an agreement listing costs for your review. If you decide to work with us, you will sign and return the agreement with the payment indicated.
First, we will need go over your c.v. in detail.. We will get as much information as we can from there, and return to you a document with highlighted blanks indicating additional information we need that is not included on your c.v..
We will complete all historical data on the applications for you, and send them to you for your final review and completion, which will basically involve:
a.) answering all “YES/NO” questions regarding regarding malpractice claims, disciplinary actions, etc. This won’t take long – just a few checkmarks.
b.) signing and/or notarizing sections indicated.
c.) having fingerprint cards completed, if applicable for the state to which you are applying. We will obtain the cards and releases from the boards and supply them to you. When the cards are returned, we will submit them for processing with the required forms and fees.
d.) attaching required photos and document copies, such as medical diplomas, training certificates, etc. We will only ask you to supply the specific documents required by the state/s to which you apply. If you don't have some of the documents needed, we'll assist you in obtaining them. Often, for example, when malpractice claims are involved, the boards require copies of court records. If you don't have them in your files, we will work with your attorney and/or the courts to obtain them for you.
e.) if applicable, supplying statements in response to any answers that on the application that require explanations, such malpractice claims or disciplinary actions. We will assist with preparing the statements on the forms or in the formats the board requires.
Alas, even if 25 boards have already received documentation verifying your background, the 26th state board to which you apply will still want new documentation on their own state forms, which must be sent directly from the schools, training programs, hospitals, references, etc. We will contact each source to find the appropriate person to handle these requests. After we send the requests, we will continue to follow up with these sources until the board confirms receipt of all of the required documentation. - FORMS COMPLETED BY HOSPITALS WHERE PRIVILEGES WERE HELD
- REFERENCE FORMS OR LETTTERS
- FORM COMPLETED BY SPECIALTY BOARD
- FORM COMPLETE BY THE DEA
- MILITARY RECORDS FROM THE NATIONAL PERSONNEL MILITARY COMMAND
- FOR FOREIGN GRADUATES, FORM COMPLETED BY THE ECFMG
- EXTENSIVE DOCUMENTATION REGARDING MALPRACTICE CLAIMS, WHICH OFTEN INCLUDES COURT RECORDS, LETTERS FROM ATTORNEYS AND/OR REPORTS FROM INSURERS
